Usage rights - in perpetuity

Terms and Conditions

I/we:

1. Give Lendlease Group Services Pty Limited (ACN 603 716 294) and each of its Related Bodies Corporate (as defined in the Corporations Act 2001) (together “Lendlease”) the irrevocable right to use my name (or any fictional name), picture, portrait, or image in all forms of media and in all manners, including composite or distorted representation, for advertising, trade, or any other lawful purposes for the period indicated above; and
2. Waive any right to inspect or approve the picture, portrait, image or finished moving footage such as DVD or video footage, sound track or written copy that may be created by Lendlease or the Photographer or the eventual use to which it might be applied; and
3. Release, discharge and hold harmless Lendlease and the Photographer, their representatives, employees, officers, agents or any person or persons, or corporation, for whom they might be acting, including any firm publishing and/or distributing the finished product, in whole or in part, from and against any liability arising from the use of my name, picture, portrait or image, DVD or video footage, sound track or written copy; and
4. Am over 18 years of age; and
5. Have read and understood this release.
6. I am the parent / guardian of the minor named above and have the legal authority to execute the release on his/her behalf.
